Documentation Criteria

All students registering with QSAS must complete the student intake form located in Ventus and submit documentation that meets the documentation criteria.  We do not recommend the submission of private and confidential medical documentation by email as it is not a secure means of transmission.

Students must provide documentation that:

  • Confirms that they are a student with a disability (disclosure of diagnosis not required). 
  • Contains information about functional limitations within the academic environment. 
  • Indicates the duration and severity of the disability or impairment. 

Students who submit documentation that meets documentation criteria may °ù±ð³¦±ð¾±±¹±ð permanent a³¦³¦´Ç³¾³¾´Ç»å²¹³Ù¾±´Ç²Ô²õ.&²Ô²ú²õ±è; Students who are unable to submit documentation that meets QSAS documentation criteria, but who are able to submit some documentation, may  °ù±ð³¦±ð¾±±¹±ð interim a³¦³¦´Ç³¾³¾´Ç»å²¹³Ù¾±´Ç²Ô²õ.&²Ô²ú²õ±è;

Accessibility Advisors may request additional documentation.  We are only able to accept documentation in English.  It is the students' responsibility to have documentation from other languages translated into English before submitting. 

 

Accepted Documentation

Examples include:

  • Attention-Deficit Hyperactivity Disorder (ADHD), Attention Deficit Disorder (ADD)
  • Neuro-diverse perspectives including, but not limited to, Autism Spectrum Disorder (ASD), Tourette Syndrome, etc.

Accepted documentation:

Regulated healthcare providers include:

  • Family physician
  • Psychologist
  • Psychiatrist
  • Nurse practitioner
  • Other trained medical physician in the area of attention/neurodiversity

Examples include:

  • Hearing loss related to, but not limited to, Deaf, deafened, hard of hearing (may be used to rule out hearing issues for individuals experiencing Central Auditory Processing Disorder)
  • Vision loss related to, but not limited to, macular degeneration; cataracts; glaucoma; diabetic retinopathy; blindness

Accepted documentation:

Regulated healthcare providers include:

  • Family physician
  • Psychologist
  • Psychiatrist
  • Nurse practitioner
  • Other trained medical physician in the area of hearing/vision

Examples include (but not limited to):

  • Dyslexia, dysgraphia, auditory processing disorders, etc.

Accepted documentation:

  • Psychoeducational assessment completed within the last 5 years or after the age of 18

Examples include:

  • Medical conditions including, but not limited to, diabetes, heart condition, cancer, Crohn’s disease, irritable bowel syndrome, etc.
  • Brain injuries including, but not limited to, concussion, acquired brain injury (ABI), etc.

Accepted documentation:

Regulated healthcare providers include:

  • Family physician
  • Nurse practitioner
  • Other relevantly trained medical physician

Examples include:

  • Major depressive disorder; generalized anxiety disorder; borderline personality disorder; eating disorder; etc.

Accepted documentation:

  • Queen's Verification Form for Mental Health
  • Neurological assessment – completed in the last 1 year
  • Psychoeducational assessment – completed in the last 5 years
  • Psychological assessment - completed in the last 1 year
  • Psychiatric assessment/report – completed in the last 1 year
  • Other medical documentation from a professional health care provider that meets the QSAS documentation criteria (confirmation of disability, functional limitations and duration/severity)

Regulated healthcare providers include:

  • Family physician
  • Psychologist
  • Psychiatrist
  • Nurse practitioner
  • Other trained medical physician in the area of mental health

Examples include:

  • Mobility impairments related to, but not limited to, cerebral palsy; developmental coordination disorder; etc.

Accepted documentation:

Regulated healthcare providers include:

  • Family physician
  • Neurologist
  • Nurse practitioner
  • Orthopedist
  • Rheumatologist
  • Other relevantly trained medical physician 

Submitting Documentation

  • Upload documentation through the .  We do not recommend submitting private and confidential documentation by email as it is not a secure means of transmission. 
  • Each type of documentation must be in one document and saved/uploaded as a PDF. 
  • Label PDF as follows: "Last Name, First Initial_Type of Documentation_Year"
